Each person going on a mission trip must complete an application. Once received, the Trips Review Committee will schedule you and your team (if applicable) for an interview. If a question does not apply to you just answer "N/A."
NOTE: Passport must have an expiration date beyond six months of the return date and have several blank pages.

Leader or Individual - please provide the church office with a copy of your airline tickets, passport, contact information here and at your destination as well as contact information for your trip insurance carrier.
